Greene House Primary Care Office Policies
PAPERWORK REQUIREMENTS
All initial intake paperwork must be completed and submitted at least 24 hours before the scheduled appointment date and time.
Failure to complete or submit required paperwork within this timeframe will result in appointment cancellation.
Incomplete paperwork will not be accepted on the day of the visit.
PAYMENT POLICY
Payment for services is due 24 hours before the scheduled appointment.
Failure to submit payment within this timeframe will result in automatic cancellation of the appointment.
Booking Requirement:
All appointments require a valid credit card stored on file at the time of scheduling.
DAY OF APPOINTMENT EXPECTATIONS
Patients are expected to:
• Log in to the telehealth platform at least 10 minutes before the appointment starts
• be in a quiet, private area
• Ensure adequate lighting for video visits
• Minimize distractions during the session. If the appointment starts and you are driving, the appointment will be cancelled, and the card on file will be charged a $50 inconvenience fee.
LATE POLICY / GRACE PERIOD
A 15-minute grace period is allowed.
If the patient has not joined the session within 15 minutes of the scheduled start time:
• The appointment will be cancelled
• A $50 late fee will be charged to the card on file
RESCHEDULING POLICY
Appointments must be rescheduled at least 24 hours in advance.
Failure to reschedule within the required timeframe will result in a:
$50 Cancellation Fee
NO-SHOW POLICY
A no-show is defined as:
• Failure to attend the appointment
• Failure to log into telehealth session
• Failure to notify the practice
Penalty:
• $50 fee per occurrence
After two (2) no-shows, the patient will be:
• Dismissed from Greene House Primary Care
• Ineligible to book future appointments
MEDICATION MANAGEMENT POLICY
A mandatory two-week follow-up visit is required for all newly prescribed medications.
This follow-up ensures:
• Medication tolerance
• Monitoring for adverse reactions
• Dose adjustments if needed
Initial Prescription Limits
Upon initial prescribing:
• No more than a 30-day supply will be issued
After the required follow-up:
• Additional refills may be provided if clinically appropriate
CONTROLLED SUBSTANCE POLICY
Greene House Primary Care does NOT prescribe controlled substances.
This includes, but is not limited to:
• Benzodiazepines
• Psychostimulants
• Narcotics
• Any Schedule II–V medications
Patients requiring controlled substance management will be referred to an appropriate specialist.
